Please read through the information below before or while completing the
form. The links are at the end of the document.
If you will please complete the information requested, it will help us to
repair your equipment properly in the shortest amount of time.
Owner: If it belongs to a company or organization, put that
name here. Use the Contact: field for the name of the
person who is most familiar with the nature of the problem.
Address: This is especially important for warranty claims.
Phone: We need to be able to contact you to deliver a cost
estimate or if more information is needed about the nature of the symptoms.
E-mail may be used if you are out of our local calling area
or if we have trouble reaching you by phone.
Physical damage: Please note any missing parts, rips,
noticeable dents or other physical damage not related to the repair.
Accessories/included items: Please include items needed to
operate the equipment such as power adapters, special disks/media, cables,
etc. Detachable power cables are not needed if they are the standard 3-prong
computer type. Any other type should be included. Remove and keep any other
items not related to the equipment. We will not be responsible for the loss
of such items.
Symptoms: VERY IMPORTANT! Tell us what we are looking for! We must be able to duplicate the
symptoms in order to repair the unit. Please describe the nature of the
failure and the settings needed to demonstrate the symptoms. (DON'T use the
word 'short'!) If the problem is intermittent ('cuts in and out') please
make special mention of it along with a description of what causes the
symptom to appear. Use the comments section if you need more space.
Item Type: Basic description of the unit: 'amp head',
'Combo', 'keyboard', etc.
Model, serial: Important in case the tag gets separated
from the unit, or if another similar unit is in the shop.
Warranty/Non-warranty: Please indicate clearly.
Warranty repairs require your original purchase receipt or a good quality
copy. If you cannot supply the purchase receipt, you will be charged for the
repair. In most cases, the warranty only applies to the original purchaser
of the new unit. Units purchased 'pre-owned' or used are not covered by
manufacturer's warranty.
Non-warranty: If the unit is non-warranty, please state
a 'call me' amount. If it appears the cost will exceed that amount, you will
be contacted for approval. (See "Repair Cost" for typical charges)
Insured value: (Write in) If you are shipping the
equipment to us, please indicate the amount you wish us to insure it for on
the return shipment. If we need to file a claim, you must be able to provide
documentation to support the amount claimed. See

Warranty Limitations: Please be aware that there are
several limitations on manufacturer's warranties. You may still be charged
in some cases even if the equipment is within the warranty period. Most
manufacturers only warrant vacuum tubes for 90 days and speakers for one
year from date of purchase. If a tube or speaker is found to have failed
after these periods, you will be charged for the diagnosis and the parts.
You may also be charged for shipping expense for warranty repair parts.
Claims for speakers may be denied if the damage appears to have been caused
by applying excessive power or if an external amplifier failure destroyed
the speaker. Manufacturers also do not cover physical damage such as broken
knobs, bent or broken control shafts, and broken jacks. The governing
question is "Was this failure caused by a manufacturing defect?" If the symptoms
cannot be duplicated at the service center, a bench test fee will be charged
to the owner. Shipping costs to the service center are the
responsibility of the owner.
Repair time: Typical repairs will take from two to four
weeks, or more if parts need to be ordered. Manufacturers require that
warranty repairs be performed using parts supplied by them, so most cases
will require that parts be ordered, increasing the repair time. If you need
the equipment sooner, additional costs may be incurred such as air freight
charges and an additional service surcharge to expedite the process. Even
under urgent conditions, timing often depends on the promptness of several
other people, so no guarantees can be made for completion by a specific
date. Equipment should be brought directly to the service center for these
cases. Contact the service center to arrange for urgent service.
Repair cost: Typical post-warranty repairs cost
approximately $100 - $175. Solid state high power amplifiers with shorted
output stages often cost $200 - $250 or more. (If it blows the fuse, this is
very likely the problem.) Please tell us the cost limit for pre-approved
repair. If it appears that it will be greater than that amount we will call
you with an estimate. Estimates are approximate and may be revised. A
declined estimate (not repaired) will be charged for at least ½ hour
($37.50-$47.50). Minimum charge is ½ hour for minor repairs. If the symptoms
cannot be duplicated at the service center, a bench test fee will be charged
to the owner, even if it is under warranty. (Manufacturers do not cover such
cases.)
